For Aliso Viejo readers: If you live or work near Town Center or anywhere along Aliso Creek Road, our Mission Viejo clinic is a seven-minute drive — close enough that the medication, the labs, and the follow-up visits fit cleanly into a workday. We hold evening hours twice a week specifically for commuters who would rather see a clinician at 5:30 than burn PTO. Costs, labs, and dose plan are walked through on visit one with the actual numbers, not a brochure.

The Aliso Viejo professional schedule is brutal on weight loss programs. You leave the house at 7:15, you are in a meeting by 8:30, and by the time you push back from your laptop the clinic’s “last appointment 5:00” sign has gone up across the entire OC medical district. That is exactly the problem we built our evening clinic block around.

The Aliso Viejo angle: what’s different about your program?
Most patients we see from the 92656 area code share three things: a salaried job inside a Town Center, Summit, or Aliso Creek office park; a calendar locked Monday–Friday between 8 and 5; and a strong preference for a clinic that does not feel like a same-day urgent care. The program is shaped accordingly.
- Evening clinic hours. Tuesday and Thursday slots run until 7 p.m. Most Aliso Viejo patients book the 5:30 or 6:00 spot on the way home.
- Telehealth check-ins after month two. Once you are stable on dose, monthly visits can move to a 15-minute video call — keeps the program going without pulling you out of work.
- Discreet billing. If your spouse, employer plan, or HSA card needs a specific line-item description for receipts, we will write it that way.
The drive from Aliso Viejo
Roughly seven minutes door-to-door from most of Aliso Viejo via Aliso Creek Road to Crown Valley Parkway, then a short hop on the 5 north. Town Center patients sometimes prefer Pacific Park Drive to Crown Valley — same time either way, less light traffic.
What does the first visit actually cover?
About 45 minutes — bookable as a 5:30 p.m. evening slot if a weekday morning is impossible. Three sections:
- History, BMI, vitals, body composition. We use bioelectrical impedance to get a starting muscle vs. fat split — important so we can track lean mass through the program, not just scale weight.
- Lab review. Basic metabolic panel, A1C, lipid panel, TSH, kidney function. Recent labs from your PCP work if drawn in the last 90 days.
- Dose and pricing plan. Standard start is 2.5 mg weekly, stepping up every four weeks based on tolerance. We walk through brand (Zepbound, Mounjaro) and compounded options with the actual monthly numbers.
Brand vs. compounded — clinician’s honest read
Tirzepatide as brand-name Zepbound or Mounjaro is the FDA-approved, manufacturer-quality-controlled product. If your insurance covers it, take that. Coverage in Orange County tech and finance employer plans is improving but still inconsistent — we have seen four otherwise similar PPO plans hand back four different answers in the same week.
Compounded tirzepatide from a properly licensed 503A pharmacy is the cash-pay route when brand is unaffordable. The active ingredient is the same molecule; the trade-off is no FDA “approved” stamp on the specific compounded formulation. We use a pharmacy with a clean compounding record and lot testing — happy to share documentation at the visit. Side effects: what evening-clinic patients ask first
The professional crowd asks one question more than any other: “Will I be functional at work?” Honest answer — yes, with a few caveats.
- Days 1–3 after a dose increase can include mild nausea, fatigue, or “brain fog.” We dose patients on Sunday or Friday evening so the worst day lands outside of high-stakes meetings.
- Constipation from week two onward — preventable with fiber, water, and magnesium glycinate (covered in our tirzepatide diet plan).
- Acid reflux at higher doses — a smaller dinner and not eating within two hours of bed fixes most cases.
The SURMOUNT-1 trial reported that 4–7% of patients discontinued due to adverse events at any tirzepatide dose — most events were mild to moderate GI (Jastreboff et al., NEJM 2022).
What is the realistic 6-month outcome?
Patients who stay on protocol typically drop 12–18% of starting body weight by month six. A 200-lb patient lands between 164 and 176 lb. That assumes hitting protein and fiber targets, two resistance sessions per week, and not skipping doses for travel or work events. Patients who stop the medication without a maintenance plan regain a meaningful share of the loss within a year, mirroring trial discontinuation patterns. The plan we discuss at visit one includes the off-ramp, not just the on-ramp.
Aliso Viejo FAQ
Are evening appointments available every week?
Tuesday and Thursday evenings run until 7 p.m. Slots fill 1–2 weeks out, so book ahead. New-patient consults and standard monthly check-ins both fit.
How is parking and entry after 5 p.m.?
Free lot parking, lit and monitored. The building is open until 8 p.m. on clinic evenings.
Can I keep my Aliso Viejo PCP?
Yes — we are an adjunct service for weight management. We will share lab results and dose history with your PCP at your request, which most patients find smooths annual physicals.
Can I do most visits over telehealth?
After the first two months you usually can. The first visit, dose-titration visits, and any labs need to happen in person.
Will tirzepatide affect my marathon or triathlon training?
Endurance athletes in our program have to be extra deliberate about fueling and protein — early satiety can collide with a 50-mile training week. Doable, but worth flagging at the first visit so we plan around the training cycle.
