How we schedule around school pickup and the 9-to-5

What does the first visit look like?

  1. 15 minutes of history. Cycles, sleep, mood, libido, energy pattern, weight history, family history.
  2. 10 minutes of labs. Phlebotomy on site. Estradiol, progesterone, FSH, total/free testosterone, SHBG, DHEA-S, A1c, lipids, TSH, free T4, free T3, vitamin D.
  3. 10 minutes for vitals and body composition. Bioelectrical impedance to capture starting muscle/fat split.
  4. 15 to 20 minutes with the clinician. Walk through what we see, what is missing, what we can start today vs. what waits for labs.
  5. Plan. Most patients leave with a starting prescription if labs and history support it, or with a clear “labs first, then we start” plan.

The Endocrine Society 2018 guideline on testosterone therapy in men supports treatment when symptoms plus two morning total-testosterone readings under 264 ng/dL are present (Bhasin et al., JCEM 2018). For women, the 2022 NAMS Position Statement supports individualized HRT for symptomatic women within 10 years of menopause or under 60 (NAMS 2022).

I am 41 and still have regular cycles. Am I too young for HRT?

Probably yes for full HRT, but perimenopause symptoms can be addressed with targeted progesterone for sleep, lifestyle work for cycle regularity, and a re-check in 6 to 12 months. We do not start estrogen replacement on a still-regular cycle.

Can I bring my kids to the appointment?

For Saturday slots, yes. The waiting area has a TV and Wi-Fi. For weekday lunch slots, the visit goes faster if the room is just for you.

Is telehealth an option after the first visit?

Yes. After the initial in-person workup and 8-week follow-up, most check-ins can run via telehealth. Labs still need to be drawn (your Lake Forest LabCorp or Quest will do).
