Our masseter Botox protocol
The masseter is one of the strongest muscles in the human body by cross-section. A meaningful dose reduction in its bulk takes more units than a forehead treatment and a different injection technique. Here is what we do:
- Dose: 25–50 units per side, depending on muscle bulk on palpation
- Technique: Three injection points per side, placed deep into the muscle belly (not superficial), away from the risorius and zygomaticus to preserve your smile
- Mapping: We have you clench, palpate the muscle borders, and mark with white pencil before injecting
- Needle: 30G, deep insertion to mid-muscle depth
- Side-to-side balancing: If your masseters are asymmetric (very common, especially with one-sided chewing or sleep clenching), we dose unevenly to match
A peer-reviewed review of botulinum toxin in temporomandibular disorders supports its use for masseter hypertrophy and TMJ-related myofascial pain (Khalifeh et al., PubMed 30922541). Aesthetic jaw slimming is an off-label cosmetic use of onabotulinumtoxinA — it is not an FDA-approved indication for masseter Botox specifically, but the use is well-established in clinical practice.
What you will feel and when
For TMJ-focused patients, we sometimes schedule a single session first, evaluate at 6 weeks, and only continue if the pain improvement justifies it. For pure aesthetic jaw slimming, we recommend planning two sessions four months apart up front. The compound effect over those two treatments is more visible than either alone.

Who is a candidate
Good candidates:
- Visibly hypertrophied masseters (you can clench and see the muscle bulge prominently)
- TMJ-related morning jaw soreness, headaches, or audible night clenching
- A square or wide lower-face shape that is muscle, not bone
- Patients comfortable with a 6–12 week timeline for visible change
Not a candidate if:
- Your lower-face width is primarily bone structure — Botox cannot reshape bone
- You have a neuromuscular condition (myasthenia gravis, ALS, Lambert-Eaton)
- You are pregnant or breastfeeding
- You are looking for instant results — this is a slow-build treatment
What your first visit covers
Plan 45 minutes. We review your jaw symptoms, dental and orthodontic history (including any prior splint or night-guard use), palpate the muscles at rest and during clench, photograph at rest and clenched from front and side, then map injection points. Injection itself runs 5–10 minutes — faster than upper-face Botox because the targets are larger and fewer.
Aftercare is simple: no chewing gum or hard chewy foods for 48 hours, no facial massage over the area for two weeks, stay upright for four hours, skip the gym today. Most patients head straight back to their day. Driving is fine immediately.

How fast will the slimming actually show up?
Subtle change at 4–6 weeks, peak slimming at 6–12 weeks. If you have a wedding or photo event, plan the injection 10–14 weeks ahead.
If it’s mainly for TMJ pain, do I need a referral?
No referral required. We do ask about your dental and orthodontic history during intake and may suggest coordinating with your dentist if you also use a night guard.
Will my smile change?
It should not, with proper injection technique. The risorius and zygomaticus muscles that drive your smile are anterior to the masseter, and we deliberately stay deep and posterior to avoid them. If your smile changes after a previous treatment somewhere else, the injection was likely too anterior.
Is masseter Botox covered by insurance?
It is not routinely covered. Some patients with documented TMJ disorders can submit for HSA or FSA reimbursement. We provide an itemized receipt — you verify coverage with your benefits administrator.
How many sessions does it take to see real slimming?
Most patients see clear results after the second session at month 4. Two or three sessions over six to nine months produce the most durable atrophy, after which maintenance intervals often stretch to every 5–6 months.
Can I combine this with upper-face Botox the same day?
Yes, very common. Many patients add forehead and 11s to the same appointment. We charge per unit, so combining areas does not multiply the visit cost beyond the units used.
