What “non-surgical facelift” actually means at our clinic
“Non-surgical facelift” is a marketing umbrella term that gets used for everything from threads to ultrasound devices. We use it more narrowly. At our Mission Viejo office, the protocol is a clinician-designed combination of two energy modalities, mapped to your specific concerns: Agnes RF for focal, precision tightening and small fat-pocket reduction, and RF microneedling for broader collagen remodeling across cheeks, jaw, and neck. Filler and toxin can be layered later, but they are not the core of this protocol.

Why South County patients consult with us for this
Mission Viejo sits in a corner of Orange County where patients tend to be sun-exposed (decades of beach, golf, and outdoor sports) and motivated to preserve, not replace, their faces. How the combination protocol works
Agnes RF (the precision tool)
Agnes uses insulated microneedles to deliver radiofrequency energy at a specific depth, sparing the surface skin while heating target tissue underneath. We use it for jowl fat pockets, submental fullness, the lower-eyelid bag, and small focal areas. It is not a full-face device. The treated tissue contracts, the small fat pockets reduce, and the skin re-drapes over the new contour.
RF microneedling (the broader tool)
RF microneedling delivers radiofrequency energy at variable depths across the full treatment area. It is for broader collagen remodeling: cheeks, mid-face, jawline, neck, and chest. It does not reduce fat. It triggers dermal heating, neocollagenesis, and gradual tightening over 12 weeks.
How we sequence them
For most full-face protocols, we run Agnes RF first on the focal areas, then RF microneedling on the broader field. Both can be done in the same visit for some patients; we split them across visits if downtime is a concern or if treatment area is large.

Areas we treat
- Jawline and jowls: Most common request. Agnes RF for the fat pocket; RF microneedling for the surrounding skin.
- Submental (under chin): Agnes RF for the fat pad; combination of RF microneedling and posture-of-tongue training.
- Neck: Primarily RF microneedling; horizontal banding responds modestly.
- Lower eyelid bags: Agnes RF in trained hands; we are selective about candidates here.
- Cheeks and nasolabial area: RF microneedling for crepiness; filler if there is volume loss.
Sessions, downtime, and results timeline
Most full-face courses are 2 to 3 sessions spaced 6 weeks apart. We re-image at month 3 and again at month 6.
Who is a strong candidate, and who is not
- Strong candidates: Late 30s to mid 50s, mild to moderate laxity, Fitzpatrick I to V (Agnes is safer across pigmented skin than many ablative options), no active skin infection, realistic expectations.
- Marginal candidates: Severe sun damage with deep elastosis, smokers (collagen response is blunted), patients on isotretinoin in the last 6 months.
- Not candidates: Active herpes outbreak at site, pregnancy, pacemaker in the treatment field (RF caution), unrealistic expectations of surgical-grade lift.
If a patient is 65, has heavy jowls and clear neck laxity, and is hoping to avoid a real facelift, we have an honest conversation. RF combinations can soften and tighten; they cannot reposition the SMAS.
What the evidence shows for RF tightening
RF and microneedling-RF have published data supporting mild to moderate skin tightening and collagen remodeling. A 2020 review in Lasers in Surgery and Medicine summarized RF microneedling outcomes for skin laxity and acne scarring with photographic and biopsy evidence of dermal collagen change (see Tan et al., 2020). Agnes RF specifically has smaller but consistent published series for periorbital and jowl applications. Realistic outcome description: modest tightening, real but not surgical, with patient satisfaction in the 70 to 85% range when expectations are set well.

Can I schedule a Friday session and be presentable for a weekend event?
For an RF microneedling-only session, often yes by Sunday evening with mineral makeup. For a combined Agnes plus RF microneedling session, we recommend 5 to 7 days of buffer before a public event with photography.
Is there parking close to the entrance?
Yes. Our building has free ground-level parking 30 feet from the front door. Helpful when you arrive numbed and would rather not walk a parking deck.
Do you treat patients from Coto de Caza and Ladera Ranch frequently?
Yes. Both are short drives (12 to 20 minutes) and are well-represented in our practice. We block longer afternoon slots specifically for combination protocols, so you do not feel rushed.
General FAQ
Will I look frozen or pulled?
No. RF tightening is collagen-driven and gradual. There is no muscle paralysis component. Friends and family typically notice you look rested without identifying a procedure.
How long do results last?
Peak result lives for 12 to 24 months, with collagen turnover continuing to fade gradually. Annual maintenance sessions extend the benefit.
Is this safer than a surgical facelift?
Different risk profile, not necessarily “safer.” Lower risk of bleeding, scarring, nerve injury; small risk of burns, post-inflammatory pigmentation, and incomplete response. Surgery has longer downtime and bigger lift; RF has shorter downtime and smaller lift.
Can I do this if I’m on retinoids?
Pause prescription retinoids for 5 to 7 days before and after each session. Over-the-counter retinols can usually be restarted at 72 hours.
Will the result get better over time?
Yes, for the first 3 to 6 months as collagen remodels, then it stabilizes and slowly fades. Maintenance once a year is the realistic plan.
Can I combine with filler or Botox?
Yes. Most patients add small-volume filler and toxin at the 3 to 4-month checkpoint, after the tightening result has set, so we can place volume on the new contour.
