The short version: NAD+ is a coenzyme every cell uses to make energy, and levels decline with age. Human evidence is still early — most rigorous data sits in small pilot trials and biomarker studies, not large outcomes trials. It is a reasonable adjunct for the right candidate, not a stand-alone anti-aging cure.

What is NAD+?

NAD+ stands for nicotinamide adenine dinucleotide. It is a coenzyme present in every living cell. Two jobs matter most for what patients feel:

  • Cellular energy. NAD+ shuttles electrons inside mitochondria so cells can make ATP — the molecule that powers muscles, neurons, and immune cells.
  • Repair and signaling. Sirtuins and PARP enzymes burn through NAD+ to repair DNA, regulate inflammation, and adjust circadian rhythm.

Tissue NAD+ falls measurably with age. Skeletal muscle and skin biopsies show roughly a 50 percent drop between young adulthood and the seventh decade, with parallel changes in immune cells (Massudi et al., PLOS ONE 2012). Chronic stress, alcohol, and metabolic disease accelerate the decline.

How do NAD+ injections work?

Honest framing: the marketing has run ahead of the data. Here is what is supported and what is not.

Supported by human data

  • NAD+ levels can be raised. Pharmacokinetic studies with NAD+ precursors (NR, NMN) consistently show increases in blood NAD+ within 2 to 4 weeks of dosing (Martens et al., Nature Communications 2018).
  • Modest cardiometabolic signals. Small randomized trials of NR have shown reductions in systolic blood pressure and arterial stiffness in older adults, though effects are modest.
  • Safety at studied doses. Up to 1,000 mg/day of NR for 8 weeks has been well tolerated in trials, with minor GI upset as the most common complaint.

Plausible but under-studied

  • Neurological support. Preclinical models show NAD+ protects neurons against amyloid and oxidative stress. Pilot human trials in Parkinson’s disease have shown biochemical changes in the brain but no clear clinical benefit yet.
  • Recovery and athletic performance. Anecdotal and short-trial signals; no controlled trial has confirmed durable performance gains.
  • Mood, focus, and sleep. Patient-reported outcomes are common in clinical practice; high-quality trials are sparse.

Not supported

  • “NAD+ reverses aging.” There is no human trial demonstrating extended healthspan or lifespan from NAD+ therapy. Animal data is promising; human extrapolation is premature.
  • “NAD+ cures addiction.” The “BR+ NAD+” detox protocol has been heavily marketed for substance use disorder. Evidence remains case-series quality.

Who is a good candidate?

NAD+ is most often a fit for:

  • Adults over 40 reporting persistent fatigue, mental fog, or slow recovery from exercise
  • Patients in post-viral recovery or peri-menopausal transitions where energy and sleep have shifted
  • Patients already running a comprehensive optimization plan (sleep, lifting, metabolic labs) who want one more lever
