What is PRP and why does the price vary so much?

PRP, or platelet-rich plasma, is your own blood drawn, centrifuged to concentrate the platelet layer, and injected into the target tissue. Platelets release growth factors that recruit local repair cells. The technique itself takes 45 to 60 minutes per session. The price range is wide because three variables change the cost: draw volume, kit quality, and provider time.

Pricing follows mid-market US norms in 2026. Orange County and other coastal-metro markets trend higher; rural and Midwest markets trend lower.

What actually drives the price?

1. Provider type and supervision

An orthopedic physician injecting a knee under ultrasound guidance bills differently than a medspa nurse injector running a face protocol. Both are legitimate; the cost gap reflects training, equipment, and time. For joint work in particular, image guidance matters and is worth the higher price.

2. Kit quality and platelet concentration

FDA-cleared closed PRP systems (single-spin and dual-spin kits) are a real per-draw cost to the clinic. Higher-concentration kits produce 5 to 7 times baseline platelet counts; lower-end kits produce 2 to 3 times. For androgenetic alopecia and tendon work, concentration matters. Ask the clinic which system they use. If they cannot tell you, that is informative.

3. Draw volume

Bigger draws (60 mL versus 20 mL) yield more concentrate, which lets the clinician treat more area or split between sites. Larger draws cost more in supplies and chair time.

4. Add-ons

PRP combined with microneedling, exosomes, or RF microneedling raises the per-session price. So does ultrasound guidance for joints. So does a topical numbing protocol. None of these are automatic; they are line items.

5. Regional rent and labor

South Orange County clinics see real estate and wage pressure that adds to the cost of the same procedure compared to inland markets.

Package pricing: what you should expect

Most clinics that take PRP seriously sell it as a series rather than per session, because the evidence base for hair and face uses 3 sessions as the studied protocol. A common package pattern:

• Hair restoration: 3-session intro package, discounted versus per-session, then a maintenance session every 6 months at single-session pricing.
• PRP facial: 3-session package over 3 to 4 months, often bundled with microneedling.
• Joints: Single session priced higher; series only if the first session yields partial relief.

Be cautious of clinics that quote one low session price and then push expensive series packages at the consult. The legitimate version of bundling is up front and itemized.

Does insurance cover PRP?

Short version: for cosmetic indications (hair, face), no. Insurance treats those as elective. For musculoskeletal indications, coverage is mixed. CMS does not cover PRP for chronic non-healing wounds outside specific research contexts, and most commercial plans treat orthopedic PRP as experimental or investigational. Some workers’ comp claims cover it. Some self-funded employer plans cover it. The realistic plan is to assume you pay out of pocket and check after the fact.

For knee osteoarthritis specifically, the published evidence is encouraging but mixed. A 2021 meta-analysis in Arthroscopy found PRP outperformed hyaluronic acid at 6 to 12 months in some pooled outcomes (see Belk et al., 2021). Patients should ask their orthopedist before booking.

Payment plans and financing

• CareCredit: Most medspas accept it. Promotional 0% APR if paid within 6 to 24 months; deferred interest if not.
• Cherry, Affirm, GreenSky: Common for larger packages. Hard credit pull for larger amounts.
• HSA / FSA: Sometimes allowed for orthopedic indications with a letter of medical necessity. Not for cosmetic.
• Clinic membership: A handful of practices offer monthly memberships that include a maintenance session per quarter.

PRP value vs alternatives

Hair: PRP vs daily minoxidil over 5 years

Topical minoxidil 5% is a modest yearly expense for two daily applications. Over 5 years, that adds up, with continuous use required to maintain benefit. A 3-session PRP series plus two maintenance sessions a year adds up over 5 years too. PRP is more expensive, but it is layered on top of (not instead of) maintenance therapy in most clinical protocols. Combination is the question, not either-or. Face: PRP facial vs microneedling alone

Microneedling alone is priced per session. PRP adds to the per-session price for the draw and processing. Whether the added cost yields meaningfully better collagen remodeling is debated. The 2024 systematic review in Dermatologic Surgery found modest but real improvement when PRP was added to microneedling for acne scarring; data for general anti-aging is thinner.

Joints: PRP vs cortisone vs hyaluronic acid

Cortisone injection: the least expensive, fast relief, repeated use can accelerate cartilage damage. Hyaluronic acid series: priced as a 3-shot series, often partially covered for knee OA. PRP: priced per session, not typically covered, longer trajectory of benefit in mild-to-moderate OA. The choice is clinical, not just financial.

How to compare clinic quotes

1. Ask which FDA-cleared kit they use and target platelet concentration.
2. Ask draw volume per session.
3. Ask whether ultrasound guidance is included for joint work.
4. Ask what is included in the package (numbing, follow-up visit, touch-up).
5. Ask about the maintenance schedule and cost beyond the initial series.
6. Get the total written out, not a “starting at” number.
